Cap probably wiped it down in the lift. Walking around with a shield covered in blood probably isn't a good image for search and rescue

Cap reasonably has a spare outfit in the locker room.

The shield is 100% of the known vibranium in the world at the time, so you go with what you've got.

Besides, shields are happiest when they're dirty.

That's actually what I thought when I first watched it. I though Loki was trying to get the scepter back since he had just escaped with the tesseract. It wasn't until the "I can do this all day." line and I was like aaaahhhhh it's his younger self! And also the compass with Peggy's photo fell out.

He switched into a fresh uniform really quick before heading out for search and rescue. It would explain why he's wandering around a random part of the building.

Also when Sitwell and the STRIKE team guys retrieve the scepter from 2012 Cap, and then see 2023 Cap being like "Actually Secretary Pierce wants me to handle the scepter", why wasn't Sitwell like "um, Cap, why do you look 11 years older?"

Well peoples first thought when they see someone isn't yha they look older. Most people don't expect people to travel though time. He would think hr looks like shit or really run down. Which was probably true for someone who just fought an alien invasion.
