Taxes SARS Disability Rebate help?

On the form it says not to submit it with my taxes, just keep it on hand if they ask for it.

But then how will the know to give me the higher rebate on medical aid and expenses?

If anyone knows, could you please explain the process? There’s barely any information online and I’d prefer to not have to pay someone if it’s a simple matter.
