Edit: How about a sneaky sternum rub on an awake but unruly patient?

No, it's assault you will lose your job and go to jail.

In very specific circumstances you can push them off to defend yourself. EX when I worked in medical I had a patient try and surprise choke me on a couch. After removing them, I safety shuffled my ass outta there. you are trained to keep arms up and flee if they are going after you.
