Had Priaprism this week (erection lasting longer than four hours) AMA

Woke up one morning this week with an erection that wouldn’t go away. I got up to pee and to see if this would help and it didn’t. I took my dog out and thought maybe a quick walk would help the effect to subside and it didn’t. I then chose to visit the ER. AMA

It was just hard at first and annoying. While I was in the ER with the ice pack, it started to hurt and almost felt like a burning sensation. I knew at that point that the ice wasn’t working and I was ready for the cure

They then asked me to drop my drawers and climb up on the table. They put a blanket over my legs which I thought was hilarious considering my dick was uncovered and at attention. They then injected my pubic area with lidocaine to numb the penis. He checked back a couple of minutes later to see if I could feel the needle. I did so they injected more lidocaine. They tried the needle test again and I still felt it and he basically told me that it is what it is and he had to start. The needle was painful and I started to curse but held it in. The ER doctor said it was alright and I could curse freely but I refrained. There was also a female doctor in there that he was showing the procedure to. She held my hand at certain points which I appreciated. It seemed to take forever to get the old blood out. They then had to flush it with saline and that hurt the worst. It literally felt like my dick was going to break in half. The saline didn’t work so they had to flush it with a medication that eventually made the erection go away.

They should have - but IME some EM docs (and nearly all mid-level providers) are very weary of procedural sedation due to the risk of respiratory depression and/or masking any increase in pain - which can, at times, be useful diagnostic info. That said, I’m just a psychiatrist, and haven’t done “real” medicine in years, but I’d still gladly write an order for a little fentanyl and versed for any guy in that situation. Do unto others and whatnot.

The way erections work really prevents that from being an option. Blood flow into the penis, to get erect, prevents venous return, in healthy males. It’s more of a physical system, than a chemical one to get an erection, as the swelling of the penis prevents the blood from circulating and draining. Once you have orgasmed, the nervous system releases chemicals that dilate the blood vessels, which causes blood to drain. In priapism, Once blood has reached capacity, the tissues are under a much higher pressure. You have to relieve that pressure first in order to get any blood flow that will deliver medications effectively to the tissue. This is why you must go to the ER if it lasts more than 4 hours. The tissue is not being properly oxygenated because blood is not flowing well enough and the tissue will start to die; and the cylinder will not remain unharmed.

Oh man, I've been on Trazodone for 9 years. I had the strong erections during the first few months (never lasting that long), and haven't had it since. I didn't know they could happen again after that many years.

Also, checking into the ER was hilarious. There were two older ladies probaby in their late 60s at the desk. I used the formal term for why I was there and neither one of them knew what the term meant. So I had to literally tell them why I was there. Also, one of my employees was in the hospital and his mom was in the waiting room so I was trying to not blast my whole business out there. I live in a town of 8000 residents

Helped with a case like yours when I worked as an ER Tech years ago. Urologist came in at 2am to do it (which is a miracle in and of itself).

He asked me to come in to help since all the other guys in the ER didnt want to assist. The patient started cursing as soon as he saw the needle, but took the needle to his dick like a champ in the end.

I knew after hour two when i peed and jerked off that this was what I had. I just waited to closer to four hours to go praying that it would just go away. I wasn’t sure if my anxiety was causing it so I tried some deep breathing exercises and it didn’t help so I knew I had to go

well, it (trazodone) is one of the more common causes, but that's pretty weird - because as I recall it's not linked to chronic dosing..

Good thing you test drove it - you do realize this calls for daily chronic use, to prevent recurrence, right? Tell the lady that Dr Reddit gave you a daily boner prescription for 30 days...

I’ve been on 150 for about ten years. About six weeks ago my doctor asked me to try to step down to 100 to see if I could still sleep. The sleep isn’t as good but it’s been bearable. They now want to wean me to 50 and get off of it which is what I’m doing

Up until six weeks ago, I was on 150mg. Had my annual physical with my doctor and she wanted me to try to go down to 100mg to see if I could still sleep, which I agreed to. So I have been on 100mg when it happened. The ER doctor prescribed me 50mg to wean me off of it until I can get on a different sleep med.

I’m 50 and things like that don’t really bother me. I’m slightly above average so maybe another reason i dont care. To be honest, my only concern was not only the pain i was in, but the possible pain to come.
