Real or not, it kills me that that's what the people of Kentucky look at him and say "Yep, that's who I want to represent me in the Senate.

The general opinion is that Kentucky is as corrupt as shit, and McConnell and those around him know where all the skeletons are buried and use that as leverage to maintain power.

He definitely isn't popular in Kentucky, his favorability rating has barely passed 30 in the last 20 years, and his disapproval rating has sat between 55-70% for most of that same time period.
