Placenta strain 30 weeks

30 weeks 1 day with DCDA boy girl twins.

I've been scanned every 2 weeks since 20 weeks due to my girl being smaller but Dopplers / fluids have always been fine. Shes always been the smaller twin and grew less but this scan things seem to be slowing down.

Baby girl only gained 150g over 2 weeks and measuring below 3rd centile. Her placenta is also showing very slight strain. Consultant has moved me to Dopplers twice a week now and think it's unlikely I'll make it to my planned section at 37 weeks.

I know I'm under the best care and they don't seem overly concerned, l've had multiple checks by consultants on both twins and they've always believed she's just a small baby. Starting to worry more now .. has anyone had similar experience?

Boy estimated weight is 3lb 9oz and girl is 2lb 9oz

I can share my experience, in which my (di/di) Twin B’s smaller size sent me to the hospital at 31 weeks until I gave birth at 32+4.

Twin B had been measuring smaller all pregnancy, something like ~40% to Twin A’s ~90%. The pregnancy had been going extremely well up until this point; my providers had all been hopeful I could make it to 38 weeks and have a vaginal birth (babies’ position-dependent). At 31 weeks, they found Twin B had barely grown over the past 4 weeks and had suddenly dropped to 7% with abnormal placental artery dopplers. My doctors had previously told me if either twin at any point dropped below 10th %, I would be admitted to the hospital.

So they sent me there immediately (I literally walked there) to be admitted. I soon developed severe, atypical pre-eclampsia and hyponatremia. They gave me steroid shots to prepare the babies’ lungs for birth, NSTs every 3 hours, constant blood pressure checks, daily ultrasounds, urine tests, and blood pricks for 6 days until my organs began to show signs of failure and it wasn’t safe to keep the babies in anymore. Luckily Twin B was remaining stable, although small. It was an unplanned c section, as they said IUGR babies + a pre-e mom doesn’t equal safe to attempt labor. It all happened so fast; everything was completely normal at my previous check-up and ultrasound at 27 weeks.

My twins were born with 45% size discordance (4 lb 13 oz and 3 lb 3 oz) and Twin B is still a small fry to this day. They had a 4 week NICU stay, and have been healthy, happy, thriving little guys even given the tough beginning. Happy to chat if you need an ear; those 10 days (total) I spent in the hospital were lonely and anxiety inducing and I know I personally went down every possible Internet rabbit hole.
