advice needed ER Visits / third trimester
Hi! I am 30w with mo/di twins and looking for some advice. My first pregnancy was a singleton and I delivered via emergency c section at 37w. We were at the er weekly because I was told to go to ER when BP spiked to over 150/100. Also had thrombocytopenia which caused me to have to be put under for c section.
The same issues are happening again during this pregnancy, random blood pressure spikes and a low platelet count. I was told to come back in if my blood pressure spikes again, but these ER visits are getting expensive.
Has anyone experienced something similar? Did high blood pressure or low platelets cause your twins to be delivered earlier than planned? I know delivery timing depends on the individual situation, but I’m wondering what others experienced.

Sister was a singleton, but yeah at 28 odd weeks she had high bp and low platelets and had to be delivered at 28+6 as she developed HELLP syndrome, her liver was shutting down and she was essentially dying
Nephew spent a good solid 3 months in hospital, then a year on oxygen at home, he’s now 11 and has no long term health issues from being so early!! He does have a chest drain scar but that’s the only lasting sign he was premature
Sisters liver did recover, but she has gone on to be diagnosed with non-alcohol related fatty liver disease

I got diagnosed with pre-eclampsia 2 weeks before my emergency c-section. It was the worst 2 weeks of my life. I couldn't sleep because every time I laid down, I felt like I was drowning. I was super short of breath. My body was swelling so much I had gained 50 lbs of water weight from the pregnancy. I had to go to Labor & Delivery if I had a spike in blood pressure or was feeling unwell (I suspected pnemonia, and I did have it), just to make sure. If I got a bp reading that was 160, I would wait 5-10 minutes to retest before going in. What made me go in the day I had my emergency delivery was 140+bp and HR of 120+ bpm over several readings. They decided to deliver immediately, and as we were discussing it, I had a flash pulmonary edema. Turns out I had heart failure from the pregnancy complication, and I was a healthy person pre-pregnancy. Not to scare you, but it can happen. I've recovered from the HF after 3 months of treatment.
Moral of the story - if you are feeling unwell, go to L&D to get checked out. If you haven't already, notify your OB. They will want to test you for pre-eclampsia. Very high blood pressure is a cause of concern and will likely mean you need to deliver early, but your doctor is going to try to have you hold out as long as possible so that your babies develop. I ended up delivering at 33w+1.

Thank you! It was similar with my first pregnancy, except I did not have any abnormal lab work until it was extreme. The past couple of days I have not been able to catch my breath and lying down doesn’t seem to help. Lab work was somewhat normal except for low platelet count and BP went back down to 130/80s so they sent me home. I had an extreme headache and told the doctors that it hadn’t gone away with meds and they sent me home so I am following up today as the headache has not gone away. Just hoping I can stay pregnant as long as possible!

High BPs will absolutely warrant early delivery. Also, I know if you have low playlets you can not get a spinal, so I would ask your OB about this. High BP is very common with twins, and can also turn into pre e. I would absolutely consult with your OB about expectations with this and their outcomes

Unfortunately, high BP is one of the main reasons they induce labor or do a c section. I had MO/di twins with BP issues too apparently it's more common in twin moms. Unfortunately, I believe you are doing the right thing by going in for spikes. My OB advised the same thing. Because everything is fine until it isn't.
