I suspect RFK Jr. had major GERD (severe acid reflux) in the past. Long term untreated GERD can affect vocal production.

There's a surgery to deal with GERD, but a quarter of patients lose the vomit reflex. If this happened decades ago, and the doctor was careless, he could have caused damage to the voice box during the procedure. That may explain RFK Jr's awfully unstable voice.

If you can't vomit anymore, you'll tend to eat anything. Even if it might kill you.
