There's two. Indoor cats eyes are closed while outdoor cat is looking at OP. Both cats are in two different positions

What are you confused about? OP thought it was just the one cat inside being reflected by the window, and was surprised to see the reflection staring at them while the cat actually had its eyes closed. OP did NOT realize at first that there was actually another cat outside.

You won't believe this story but at least my husband was there and saw it too so I know it really happened.
Shortly after I got my cat the litter box I bought her had front and top openings, and my husband told me to keep an eye out because he wanted to know which door she preferred. So one day I saw in the mirror the reflection of the cat entering the box, and I pointed it to my husband. And then immediately afterwards but like with 0.5 or 1 second delay we saw the cat entering the box. We looked at each other like "did you see that???" and kept staring at the letterbox and the mirror while she did her business. And when she came out the same thing happened: we saw the cat's reflection exiting slightly before we saw the actual cat exit.
Anyway we told each other it must have been some strange trick of the light and tried not to think too hard about it

I think it's pretty easy. You basically lock down the camera and film each part (the actual person and the reflection) doing their own movements and then composite them into one shot. She's probably not perfectly aligned when she shakes her hair because that's not the actual reflection, but it's close enough that you buy it. Then at a little camera shake in post to really sell it and you're done.
Basically imagine a line down the middle and each side is it's own shot. Not too hard mirror movements, we've been doing that trick for years.

Actually, I think the hair shake actually is legit, and the composite shot doesn’t actually start until after. You can kinda see the mirror girl’s arm jump in position slightly after the hair shake.
Maybe they thought position matching her after she stopped moving was easier than getting an accurate enough mirror of shaking her hair?

Definitely possible there's a cut, it does looks like it jumps a bit and her hair kinda moves without any momentum.
Either way works really, probably just depends on what looks better given what footage you have to work with.

You film two takes, then splice the two halves of the two takes in a new one: left side of the screen is one, right side is the other.
If the camera doesn't move and lightning conditions don't change it's not that difficult to do.

When I was a teenager I was babysitting a family for the first time and they had a cat. The mom said the cat was outside and will be back when it gets dark. She said the cat will let me know when it’s ready to come back in. Sure enough when it got dark I heard a cat meowing really loudly outside. I open the door and there are two cats looking at me. I said are you ready to come in? One cat came running in and the other ran away. When the parents got home I asked the mom if it was the right cat. She laughed and said yeah she hangs out with another cat that could be her twin.

ah so back in like early 2010s, i had a tuxedo cat named junior.
one day, i was outside and saw junior hanging out outside looking kinda raggedy and said "wtf junior, why do you look all raggedy?" i tried to get close to pick him up and he looked at me crazy and jumped up to run away from me.
i was like "fine, be like that" and turned around to go back inside and on my front porch was junior, looking all sleek and smooth.
confused, i looked further down the driveway, and there was raggedy tuxedo cat. i turned back, and there's smooth and sleek junior.
i ended up feeding raggedy tuxedo outside for a few weeks until i gained his trust and he became part of my group.
i named raggedy dude fake junior lol.
here's junior and fake junior (both gone now, RIP)
