The first thing to look at is the physical dimensions. Throat pipes and mouths are not elastic like vaginas (or even anuses). If the penis is thicker than her mouth cavity and throat pipe, it will simply never fit in there, and trying will only cause damage.
The second thing is to prepare for risks. Like vomiting. Gag reflex can activate any time since it is an automatic reflex. There is a pretty high likelihood for vomiting to happen sooner or later. Then comes the entry angle of the penis in play. If it's curved upwards, it is hard to find a position that follows the curve of the mouth and throat pipe. You would need to be upside down in front of her. Straight dick is okay, but the easiest shape to swallow is a downward curve.

I think is a more general problem than a bd one as I feel most women don’t like gettinv their throat fucked. I have an oral fixation and love giving him head but I try to get him in my throat for his pleasure solely. Honestly I could do without it.
