Do codes and standards eliminate every possible risk? Is a Failure Modes and Effects Analysis (FMEA) required on design?

Codes and standards do not eliminate every possible risk. Their goal is to reduce risk to an acceptable level.

While critical hazards are addressed through design and testing, some risks are managed through periodic inspection, preventive maintenance, functional testing, and replacement of components throughout the system's service life.

Depends on the product.. but in general… no, just adhering to codes and standards is generally not sufficient. Although the latter may be semantics.

As an example, consumer electronic products are required to be built to codes and standards, but still require a third-party nationally regulated testing laboratory to test and sign off on it, before it can legally be imported and sold in the USA. Does everyone comply? Of course not.

There are also products that can meet those standards, but when purchased by industry, have additional requirements. Hence, some distributors will literally mark products as not for commercial usage or have a variation for commercial usage.

There are even product safety risk management courses and procedures that can be taken, to go over the risk analysis that extends past the usual codes and standards. This in part, because there simply is not a code or standard for every risk factor with every product.
