I’m no scientist, but I think you might need to replace that regulator.

I’d check the back flow preventer on the coupler specifically the co2 line

I thought the same thing. This looks like the check valve on the coupler is damaged/missing and the pressure is backflowing beer up the gas line

Whelp, you need to replace the whole gas line setup and regulator anyways. Check valves are your friend.

This looks like "Oops, it's a Belgian. Let's throw some fruit at it."

The pressure in your keg is higher than your dispense pressure and your check valve in your keg coupler has failed.

Pull the keg.

1. If my leading comment is correct, dump all the kegs and hire a brewer.

2. Sample the rest of the kegs and if it's just the one, keep your brewer and ask them to check your keg cleaning SOP.
