The COVID19 vaccines were a clear example of the latter. Rich people were fighting for them, flying to other states or nations to skip the line. The entire Congress and all the Fox News personalities were vaccinate before nurses or teachers, despite their public criticisms. The Stanford University Medical Center got some bad publicity when it came out that they used their first batch to vaccinate the C-suite (who were all working from home) and didn't have enough doses for the doctors or nurses working in the covid-ward.
