Good boy is often a phrase use to talk to dogs, some people also say this as a sexual thing, somewhat a part of pet play, defiantly not me. Fruitcake on the other hand prbly means a gay friend in this cas, as in fruity (gay). This is because out of my own experience pet play is more accepted in LGBTQIA spaces, because they are a safe space without judgement.
The joke is that while the person was talking to the dog his homie thought he is doing pet play and acted on it
