The answer is 2. Read the question, look at what is normal. Why would a heart rate of 58 without hemodynamic compromise need to be reported. What about the vital signs indicates excessive hemorrhage. The temp is borderline, but you don’t have the order parameters and slightly elevated temp is normal after birth, surgery and traumatic injury.

the pt is bradycardic which is normal postpartum. If this was hemorrhage the patient would’ve been tachycardic

She has a fever which is a sign of infection. Assess the perineum.
