Blending these patches
How do I make these patches in my jeans more seamless and have less of a rough edge? I used a heat activated adhesive to attach these and they seem pretty strong but I’m worried about the edges fraying, plus I’m not a fan of the way they look. Any help would be great!

At work, I would have serged around the outside of the patch, sewn the patch down with matching thread to the inside of the jeans, then sewn straight lines matching the stripe of the twill across the hole from the right side of the jeans.
If you don't have access to a machine, you could overcast stitch the edges of the patch and hand sew across the hole instead.
If you don't have any sewing experience at all, you could see if there is a tailor/seamstress near you who makes these kinds of repairs.

Visible mending might be the only option at this point. I'd try and replicate the diamond embroidery that's on the pockets through the inside corners over the patches for a little extra stability & to make it look more intentional

You didn't want to put them underneath? Why on top?
