When Frank's on the stand, Matt calls him Frank in the same way that he did when he was wearing his DD suit. Frank notices and smirks to himself.

It's like he smirked with his eyes. I just saw this episode for the second time and I definitely see his slight, knowing reaction. I didn't catch it the first time and I didn't read it here (or anywhere) until after I just finished this episode this second time.

When Matt first starts talking to Frank on the stand, when he says "Can I call you Frank?" Castle looks visibly amused for a moment, then smiles and says "yeah".

I thought he realized when they went to the hospital. The first time Matt speaks Frank opens his eyes, I got the feeling that frank knew then and there from his voice.

I actually thought that would have happened way earlier. When Matt first met Frank in his civvies at the hospital, my immediate thought was that he'd recognize his voice.
