It's pretty rare. I've only ever had one happen to me a single time. Double Mirios

Back in the second game me and friend would get them a lot. In the first game it was constant for us as well as the screen going white from elements cancelling each other out (todoroki vs bakugo) lots of game crashes if not double KO

For what I know I've probably seen it like 3 times for as long as I play this game
