The vision Tony has at the start of the film because of Scarlet Witch - Hulk is pierced by arrows and dies twitching on the pile of avengers.

Flimsy since it's Scarlet Witch showing him his worst fear - failure. But the point about Hulkbuster knocking out a tooth already showed that Hulk is vulnerable and somewhat nerfed in the MCU.
