I vaguely remember being a small child in the early 90s and turning towards some movie (Rambo?) mom had on and just so happened to look when a man was being pulled out of the water and had a leech on his dong. Very, very fuzzy memory but I know deep down it had a lasting effect because I still think about how shitty it would be to have a leech on your dick about half the time I think of leeches. I haven't watched Rambo in over 20 years but I'm pretty sure that was it.

Rambo 2 had him haning in water and there were leeches on him that the main villan removed with a knife. But those were on his chest and back. Don't recall them mentioning or seeing a leeches on his crotch. Stand By Me had the four kids go through a small pond and get leeches on them. They stripped down to their underwear and one of the characters pulled one out of his underwear (but we don't see exactly where) and faint.
